SMA® Gold Prem Breast Milk Fortifier is a nutritional supplement to be added to expressed breast milk for the dietary management of preterm low birthweight babies. It helps to provide the particular nutrient requirements of preterm low birthweight babies including extra protein, energy, calcium, omega 3 and 6, and vitamins A and E. Breast milk supplemented with SMA® Gold Prem Breast Milk Fortifier better meets the nutritional needs of low birthweight preterm infants than unsupplemented breast milk. SMA® Gold Prem Breast Milk Fortifier should only be used under the medical supervision of a healthcare professional. It is not suitable for use in infants with diagnosed cows’ milk protein allergy, lactose intolerance and metabolic congenital diseases such as galactosaemia/glucose-galactose malabsorption or for vegetarian diets.

Preparing your baby's feed

Whilst this product is made under strict hygienic conditions, it is not sterile. Failure to follow instructions on preparation and storage may make your baby ill.

    Step 1

    Check best before date. Wash hands well. Wash and sterilise all feeding equipment according to manufacturers’ instructions.

    Only prepare one feed at a time. Gently swirl the container of the expressed breast milk (EBM).

    Step 3

    Pour 25 ml of EBM into a sterile container. Before mixing with fortifier, warm up EBM to body temperature. DO NOT use a microwave to prepare or warm feeds.

    Step 4

    Separate and add only one sachet (1g) of SMA® Gold Prem Breast Milk Fortifier. Do not attempt to mix part of a sachet.

    Step 5

    Place cap on container, gently shake the container until the powder is fully dissolved. Feed immediately.
    SMA® Gold Prem Breast Milk Fortifier can be given from a bottle or via a naso-gastric tube. Discard any milk remaining after each feed.

25 ml 1 sachet (1 gram)

*It is recommended to proceed gradually, starting with 1 sachet (1 g) SMA® Gold Prem Breast Milk Fortifier per 50 ml of breast milk. After evaluating tolerance over a 24 hour period, fortification can be advanced to full strength (4 g) SMA® Gold Prem Breast Milk Fortifier/100ml of breast milk or follow hospital guidelines on feeding.

The minimum quantity of breast milk for use with one sachet (1 g) of SMA® Gold Prem Breast Milk Fortifier is 25 ml.

Important feeding information

  • We recommend preparing each feed as required. Opened sachets should be used immediately or discarded. Use according to normal hospital procedures for handling breast milk.

  • Expressed breast milk can be frozen and fortified once thoroughly defrosted. If frozen breast milk is used, gradually defrost in refrigerator (under 4 ºC). DO NOT use a microwave oven or allow to defrost at room temperature. Ensure thorough mixing prior to feeding.

When using the product with very low birthweight infants (VLBW – 1.5 kg or less):

  • Regularly monitor and assess blood nutrient levels.

  • For enteral use only.

  • Do not use any other nutritional supplements to fortify breast milk unless medically indicated.

  • It may not be necessary to fortify breast milk collected less than 2 weeks post-partum, as it is particularly rich in nutrients.

Caution

Special considerations when using this product.

  • When possible it is recommended that expressed breast milk be analysed in order to define a corresponding fortification strategy.

  • For tube feeding: once fortified breast milk is prepared, do not keep at room temperature longer than 4 hours.

  • If using banked breast milk, ensure that it is pasteurised.

SMA Gold Prem® 2 is a special catch-up formula intended for the dietary management of preterm low birthweight babies who are not solely fed breast milk. Preterm is defined as infants born at or before 37 weeks of pregnancy. Infants who are born prematurely weight much less than term infants, this is because a significant amount of baby’s weight gain occurs between 36 and 40 weeks of pregnancy.

Preparing your baby's feed

    Wash hands well.

    SMA Gold Prem® 2 can be fed at room temperature. If required to heat, stand the bottle in warm water.

    Step 3

    Shake bottle well and remove cap and aluminium foil. Unwrap SMA® NUK® orthodontic pre-sterilised teat and holding the teat ring, screw directly onto the bottle.

    Step 4

    If warmed, test temperature by shaking a few drops of milk onto the inside of your wrist – milk should be lukewarm.

Preparing your baby's feed

    Step 1

    Pull the safety seal and open the lid. The scoop is provided under the lid. Lift the tab, pull on the foil seal, carefully and safely remove the seal and dispose of it.

    Wash hands well. Wash and sterilise all utensils according to manufacturers’ instructions.

    Boil 1 litre of fresh tap water. Allow boiled water to cool for no more than 30 minutes. Measure the required amount of water (see feeding guide) into a sterilised bottle, carefully – the water is hot. Do not use artificially softened or repeatedly boiled water.

    Step 4

    Using only the scoop provided, add the correct number of scoops of powder (see feeding guide), levelling off each scoop with the back of a clean dry knife. Store the scoop in suspension inside the can and replace lid.

    Place the sterilized teat and cap on the bottle and shake well until powder is fully dissolved. Cool bottle under cold running water or in a bowl or jug of cold water until lukewarm, do not immerse the teat.

  • Do not add extra powder or water to make feeds stronger or weaker and do not press powder into scoop. Using too much or too little powder can make your baby ill.

  • We recommend preparing each feed in individual bottles when required.

SMA Gold Prem® 1 is a nutritionally complete preterm low birthweight formula for the dietary management of preterm low birthweight babies. It provides higher levels of energy, protein, vitamins and minerals compared to typical infant milks and is specially formulated to help support their immature body systems.

Preterm is defined as infants born at or before 37 weeks of pregnancy. Infants who are born prematurely weight much less than term infants, this is because a significant amount of baby’s weight gain occurs between 36 and 40 weeks of pregnancy.

SMA Gold Prem® 1 should only be used following recommendation by a healthcare professional. Your healthcare professional will advise you when to move from SMA Gold Prem® 1 to another formula.

Preparing your baby's feed

Failure to follow instructions on preparation and storage may make your baby ill.

    Wash hands well.

    SMA Gold Prem® 1 can be fed at room temperature. If required to heat, stand the bottle in warm water.

    Shake bottle well and remove cap and aluminium foil. Unwrap SMA® NUK® orthodontic pre-sterilised teat and holding the teat ring, screw directly onto the bottle.

    If warmed, test temperature by shaking a few drops of milk onto the inside of your wrist- milk should be lukewarm.

Important feeding information

  • Strictly follow hospital procedures

  • Check best before date

  • Wash hands well, shake bottle before use, remove lid and aluminium foil and attach a hygienic teat.

  • If required to heat, stand the bottle in warm water.

  • Feed immediately after opening. For hygienic reasons, discard unfinished feed as soon as possible, and always within one hour.

  • After use discard bottle, teat and remaining contents.

  • For tube feeding, once the product is prepared it should be used within 4 hours.

  • Ensure all feeding equipment is sterilised before use.

  • For enteral use only.

  • Do not alter or add to formulae unless medically directed

  • Do not microwave, hot spots may occur and cause scalding.

  • Do not freeze.

Nutritional requirements of low birthweight infants vary accordingly to the age, size and clinical condition of each infant. Enteral feeding of SMA Gold Prem® 1 should be initiated in small volumes and gradually increased as tolerated. Feeding intolerance can indicate a serious condition; at the onset of symptoms, enteral feeding rate should be slowed or discontinued until the symptoms resolve. Broad ranges for recommended intake are: volume 135-200 ml/kg/day, energy 110-135 kcal/kg/day, protein 3.5-4.5g/kg/day.
